Lake Elementary Kindergarten and PreK Registration

Parents/Legal Guardians: The following information is required for PK & Kindergarten Registration:
• Child’s ORIGINAL Birth Certificate
• Up-to-date Immunization/shot records
• Child’s Social Security card
• Proof of Residence (Electric or Gas Bill with the physical service address)
• Proof of Income Pre K only (W2 Form, and 2 recent pay stubs)
Those entering Kindergarten should be five years of age by September 30, 2015.
Those entering Pre K should be four years of age by September 30, 2015.
***Public PreK Programs are state and federally funded and income based; therefore, there are limited openings available at each site. Completing an application does NOT guarantee placement.***

• Child’s Birth Certificate (original)
• Immunization/shot records (up to date)
• Child’s Social Security card (original-not just the number)
• Proof of Residence (Electric/Gas Bill or current Lease Agreement, with the physical service address- in mom or dad’s name) NO CABLE OR PHONE BILLS Accepted
• Proof of Income (W2 Form, and 2 recent pay stubs) (PK Only)
• Parent Driver’s License
• Custody Papers if applicable
If your last name begins with A-H, please be at registration for 9:00 AM.
If your last name begins with I-Q, please be at registration for 10:00 AM.
If your last name begins with R-Z, please be at registration for 11:00 AM.
Those entering Kindergarten should be five years of age by September 30, 2015.
Those entering Pre-K should be four years of age by September 30, 2015.
*** Kindergarten Parents: You can access all registration papers on line at www.apsb.org under Central Primary/School Information. Please have all papers filled out when you arrive for faster service.

Galvez Primary Registration for Pre-K & Kindergarten
On Monday, March 30, 2015, from 9:00 a.m. to 12:00 p.m., Galvez Primary will have registration for Pre-K & Kindergarten. Those entering Pre-K MUST BE FOUR (4) YEARS OLD by September 30, 2015. Those entering Kindergarten MUST BE FIVE (5) YEARS OLD by September 30, 2015.
Registration will be held in the Kindergarten building at Galvez Primary School. Please sign-in at the front office and get a visitor’s badge. Space will be EXTREMELY limited and there is a lot of paperwork to be completed, so PLEASE DO NOT BRING the child being registered or ANY OTHER CHILDREN.
In order to avoid long lines, please come to the Galvez Primary Kindergarten Building at the designated times listed below:
9:00 -10:00 Last names beginning A-G
10:00 -11:00 Last names beginning H-M
11:00 -12:00 Last names beginning N-Z
To complete Kindergarten registration, you MUST BRING the following to school. You will NOT be allowed to register your child without the following:
Child’s birth certificate
Child’s social security card
Child’s up-to-date immunization record (Including after age 4 shots)
Proof of residence (CURRENT Electric or Gas bill – only electric, gas or rental lease with rent receipt.) If you are residing in someone’s home, a residency affidavit will need to be completed prior to registration. These can be picked up in the office at Galvez Primary.
To complete Pre-Kindergarten registration, you MUST BRING the following to school. You will NOT be allowed to register your child without the following:

Income verification - W2 Form, and 2 recent pay stubs
All Pre-Kindergarten registration applicants must complete an application form online. Pre-K Registration will proceed faster if you access HYPERLINK “http://www.ascensionearlychildhood.org” www.ascensionearlychildhood.org and fill out the application in advance. All documents, such as birth certificate, social security card, etc., can then be uploaded to the website for you at registration.
Current Pre-K and Kindergarten students WILL NOT have school on March 30, 2015. First grade through fifth grade students will attend school this day.
