Capital Area Human Services District (CAHSD)

REQUEST FOR PROPOSALS FOR

Assertive Community Treatment (ACT) &

 Forensic Assertive Community Treatment (FACT)

CAHSD, a political subdivision of the State Of Louisiana, is conducting a fair and impartial competitive procurement process to request proposals from qualified, licensed contractors interested in providing Assertive Community Treatment services, with the ability to link with the justice system for the provision of Forensic Assertive Community Treatment services. These services will be especially crucial as the system of care in Louisiana shifts from institutionalization to community based services, providing the needed link between traditional services and inpatient level of care.

The services contracted as a result of this Request for Proposals (RFP) will be directed by CAHSD and provided to identified consumers within the seven (7) parishes served by CAHSD: East and West Baton Rouge; East and West Feliciana; Iberville; Ascension; and Pointe Coupee. Qualified contractors must be licensed by the State of Louisiana to provide any services that require a license.  Non-profit and for-profit agencies may submit proposals in response to this RFP, which is made possible by CAHSD.

The contract period will be renewable annually for up to 3 years, beginning July 5, 2010.  A full summary of the work specifications is contained in the Proposal Guidelines package which will be available beginning May 27, 2010 through June 28, 2010. 

The last date for CAHSD staff to receive questions concerning the RFP will be June 25, 2010.

The package may be picked up at: CAHSD Administration, 4615 Government St., Bldg. 2, Baton Rouge, LA 70806 between the hours of 8:00 am and 4:00 pm, Monday through Friday, except for state holidays or it can be obtained from the website below.  Offers must be submitted in the format specified in the Proposal Guidelines document in order to receive consideration.

Permanent Supportive Housing Initiative

Housing Support Team Services

CAHSD, a political subdivision of the State Of Louisiana, is conducting a fair and impartial competitive procurement process to request proposals from qualified, licensed contractors interested in providing PSH HST Services through the provision of staff with flexible work schedules to provide the following PSH activities:  Tenant Services Liaison; Pre-Tenancy Assistance; Move-in Assistance; Tenancy Stabilization; and Service Coordination.  The selected contractor also provides crisis prevention/intervention planning/response, non-emergency transportation services, and serves as the fiscal agent for the CAHSD PSH Housing Establishment & Preservation System (HE&PS). 

The services contracted as a result of this Request for Proposals (RFP) will be directed by CAHSD and provided to the identified PSH consumer households within the seven (7) parishes served by CAHSD: East and West Baton Rouge; East and West Feliciana; Iberville; Ascension; and Pointe Coupee. Qualified contractors must be licensed by the Louisiana Department of Health and Hospitals to provide Case Management services or Supervised Independent Living services.  Non-profit and for-profit agencies may submit proposals in response to this RFP, which is made possible by CAHSD.

The following resolution was offered by Dennis Cullen and seconded by Dempsey Lambert:

RESOLUTION

A resolution declaring the intention of the Parish of Ascension, State of Louisiana (the "Issuer"), to issue its Sewer Revenue Bonds in an amount not to exceed Eighteen Million Dollars ($18,000,000) for the purpose of constructing and acquiring improvements, extensions and replacements to the sewer system of the Issuer; generally describing said Sewer Revenue Bonds and the security therefor; authorizing the newspaper publication of a notice of such intention setting forth a date and time when said governing authority will meet in open and public session to hear any objections to the proposed issuance of such Bonds; providing for the filing of a petition by a certain percentage of the electors of the Issuer objecting to the issuance of the proposed Bonds unless an election is held on the question of the issuance thereof; applying to the State Bond Commission for the approval of the contents of the proposed notice of intention; and other matters in connection therewith.

WHEREAS, the Parish of Ascension, State of Louisiana (the "Issuer"), proposes to construct and acquire improvements, extensions and replacements to its sewer system, a work of public improvement of the Issuer (the “System”), and proposes to finance a portion of the costs thereof through the sale and issuance of not exceeding Eighteen Million Dollars ($18,000,000) of Sewer Revenue Bonds of the Issuer (the "Bonds"); and

WHEREAS, the Issuer proposes that the Bonds will be issued in the manner prescribed by and under the authority of Article VI, Section 37 of the Constitution of the State of Louisiana of 1974, as amended, Part XIII of Chapter 4 and Chapters 13 and 13-A of Title 39 of the Louisiana Revised Statutes of 1950, as amended (collectively, the "Act"), and other constitutional and statutory authority; and

WHEREAS, the Issuer, in accordance with the provisions of the Act, now desires to authorize the giving and publication of notice of intention generally describing the Bonds and the security therefor, setting forth a date and time when they will meet in open and public session to hear any and all objections to the proposed issuance of the Bonds, providing for the filing of a petition by a certain hereinafter described percentage of the electors objecting to the issuance of the Bonds unless an election is held on the question of the issuance thereof, and to apply to the State Bond Commission for approval of the contents of the proposed notice of intention; and

NOW, THEREFORE, BE IT RESOLVED by the Parish Council of the Parish of Ascension, State of Louisiana, acting as governing authority of said Issuer, that:

SECTION 1.  Declaration of Intent.  Pursuant to and in compliance with the provisions of the Act, and other constitutional and statutory authority, the Parish Council of the Parish of Ascension, State of Louisiana, acting as the governing authority of the Issuer (the “Parish Council”), does hereby declare its intention to issue not exceeding Eighteen Million Dollars ($18,000,000) of Sewer Revenue Bonds of the Issuer, in one or more series, on a taxable or tax-exempt basis, the proceeds of which will be used to: (i) construct and acquire improvements, extensions and replacements to the System, including but not limited to, a new treatment plant and lift station (the “Project”) and (ii) pay the costs of issuance of the Bonds.  The Bonds shall be limited and special revenue bonds of the Issuer, secured by and payable in principal, interest and redemption premium, if any, from the income and revenues derived by the Issuer from the operation of its sewer system (the "System"), after paying the reasonable and necessary costs and expenses of operating and maintaining the System and other available funds of the Parish (collectively, the “Revenues”).  The Bonds shall not constitute an indebtedness or pledge of the general credit of the Issuer.  The Bonds shall be of such series, bear such date, mature at such time or times (not to exceed twenty-two (22) years from their date of issuance), bear interest at such rate or rates (not exceeding .95% per annum), be in such denomination or denominations, in fully registered form, carry such registration privileges, be payable in such medium of payment at such place or places, be subject to such terms of redemption and be entitled to such priorities on the Revenues set forth in an ordinance adopted by the Issuer, authorizing the issuance of the Bonds.

The Issuer will, in such ordinance, enter into such covenants with the future owner or owners of the Bonds as to the management and operation of the system, the imposition and collection of rates and charges of the services rendered thereby, the disposition of such fees and revenues, the issuance of future bonds and the creation of future liens and encumbrances against the System and the Revenues, the keeping of books and records, and other pertinent matters as may be deemed proper by the Parish Council.  Such ordinance will also include remedies in case of default, provisions for the issuance of parity bonds, and such additional covenants, agreements and provisions as are judged advisable or necessary by the Issuer and the purchaser of the Bonds, including sinking funds and reserves for the payment of principal and interest on the Bonds and an adequate maintenance or contingency fund for those repairs and improvements to the System as may be necessary to assure adequate and efficient service to the public, all as provided by the Act.

SECTION 2.  Sale of Bonds.  The Bonds will be sold at public or private sale, as provided for in the Act and statutory authority.

SECTION 3.  Public Hearing.  The Parish Council will meet in open and public session on a date and time to be determined, after required notice, to hear any objections to the proposed issuance of the Bonds; provided, however, if at such hearing a petition or petitions duly signed by the electors of the Parish in an aggregate number not less than five percent (5%) of the number of the electors of the Parish voting in the last special or general election held in the Parish object to the issuance of the Bonds, then the Bonds shall not be issued until approved by a vote of a majority of the qualified electors of the Parish who vote at a special election held for that purpose in the manner provided by Title 18 of the Louisiana Revised Statutes of 1950.  Any such petition shall be accompanied by a certificate of the Ascension Parish Registrar of Voters certifying that the signers of the petition are registered electors of the Parish and the number of signers amounts to not less than five percent (5%) of the registered voters that voted in the last election held in the Parish, all as provided by the Act.

SECTION 4.  Form of Notice of Intention.  The Parish President and the Chairman and Secretary of the Parish Council are hereby authorized, empowered and directed to publish an appropriate notice of the intention of the Issuer to issue the Bonds in accordance with the provisions of this resolution and the Act.  Such notice of intention shall embody in a general way substantially all the provisions of this resolution herein above set out and shall be published in four (4) consecutive weekly issues of The Gonzales Weekly Citizen, a newspaper of general circulation and published weekly in the Parish of Ascension, and shall be in substantially the form attached hereto as Exhibit A, with such alterations as the Parish President, upon advice of Bond Counsel, may deem necessary and appropriate.

SECTION 5.  State Bond Commission.  Application be and the same is hereby formally made to the State Bond Commission, Baton Rouge, Louisiana, for approval of the contents of the notice of intention attached hereto as Exhibit A.  A certified copy of this resolution shall be submitted to the State Bond Commission, together with a request for prompt consideration and approval of this application.  The State Bond Commission will be requested to give its final approval to the issuance of the Bonds prior to the delivery thereof.

SECTION 6.  Declaration of Official Intent under Reg. 1.150-2.  Prior to the issuance of the Bonds, the Issuer anticipates that it may pay a portion of the costs of construction and acquiring improvements, extensions and replacements to the System, and costs related thereto, from other available funds.  Upon issuance of the Bonds, the Issuer reasonably expects to reimburse said expenditures from the proceeds of the Bonds.  Any such allocation of the proceeds of the Bonds for reimbursement will be with respect to capital expenditures (as defined in Treasury Regulation 1.150(h)) and will be made upon the delivery of the Bonds and not later than one year after the later date of (i) the date such expenditure was made or (ii) the date the improvements were placed in service.  This resolution is intended to be a declaration of intent to reimburse in accordance with the provisions of Treasury Regulation 1.150-2.

SECTION 7.  Employment of Bond Counsel.  The Issuer hereby finds and determines that a real necessity exists for the employment of special bond counsel in connection with the issuance of the Bonds, and accordingly Dugas LeBlanc & Mire, L.L.C., Bond Counsel of Donaldsonville, Louisiana, is hereby employed to do and perform comprehensive legal work with respect to the issuance of the Bonds.  Said special bond counsel shall prepare and submit to the Parish Council as to the issuance and sale of the Bonds, and shall furnish their opinion covering the legality of the issuance thereof.  The fee of special bond counsel in connection with the issuance of the Bonds is hereby fixed at a sum not to exceed the maximum fee allowed by the attorney General's fee guidelines of revenue bonds based on the principal amount of the Bonds actually issued, sold, delivered and paid for, plus "out-of-pocket" expenses; provided, however, that said fee shall be contingent upon the sale, issuance and delivery of the Bonds.

SECTION 8.  Approval of Legal Fees by Attorney General.  A certified copy of this resolution shall be submitted to the Attorney General of the State of Louisiana for his written approval of said employment and of the fees herein designated as required by law.

SECTION 9.   Employment of Financial Advisor.  Government Consultants of Louisiana, Inc. is hereby employed as Financial Advisor to the Issuer and will coordinate professional work with respect to the issuance of the Bonds.  The compensation to be paid  to the Financial Advisor shall be in such amount approved by the Issuer and the State Bond Commission and shall be contingent upon delivery of the Bonds.

SECTION 10.   State Bond Commission Policy on Swaps.  By virtue of the Issuer's application for, acceptance and utilization of the benefits of the Louisiana State Bond Commission's approvals resolved and set forth therein, the Issuer resolves that it understands and agrees that such approvals are expressly conditioned upon, and it further resolves that it understands, agrees and binds itself, its successors and assigns to, full and continuing compliance with the "State Bond Commission Policy on Approval of Proposed Use of Swaps, or other forms of Derivative Products Hedges, Etc.," adopted by the Commission on July 20, 2006, as to the borrowing and other matters subject to the approvals, including subsequent application and approval under said Policy of the implementation or use of any swaps or other products or enhancements covered thereby.

This resolution having been submitted to a vote, the vote thereon was as follows:

STATEMENT OF WORK:

To construct roadway widening and drainage improvements along West Worthy Rd. from Boudreaux Road to the Gonzales City Limits. The type of construction being grading, drainage structures, cement stabilized base course, asphaltic concrete pavement, aggregate shoulders.

All Bids must be in accordance with the Contract Documents on file at the Parish of Ascension, Public

Works Department, 42077 Churchpoint Road, Gonzales, Louisiana 70737.

Copies of Specifications, Bid Documents, Contract Documents and Construction Plans for use in preparing Bids may be obtained from the Glenn G. Shaheen & Associates, Inc., 1022 S. Purpera Avenue, P.O. Box 298, Gonzales, LA 70707-0298, (225) 644-5523 upon payment of One Hundred Fifty Dollars ($150.00) dollars per set made payable to Glenn G. Shaheen & Associates, Inc. Documents can be mailed to bidders for an additional $5.00 per set. No refunds will be made for returned plans.

Where bids are to be received on forms furnished by the awarding authority, no contract documents shall be issued to anyone except a Licensed Contractor or his authorized Representatives. In no event shall any documents for bidding be issued later than twenty-four (24) hours prior to the hour and date set for receiving bids.

Each bid must be submitted in a sealed envelope bearing on the outside the name of the bidder, his/her address, contractor's state license number and the name of the project for which the bid is submitted. If forwarded by mail, the sealed envelope containing the bid must be enclosed in another envelope addressed to the Ascension Parish Purchasing Department, 120 East Railroad Street, Gonzales, Louisiana (P. O. Box 2392, Gonzales, Louisiana 70707), mailed certified mail and must be received no later than the bid opening.

Contractors desiring to bid shall submit to the Engineers, with their request for Contract Documents, contract documents deposit and evidence that they hold State License of proper classification and in full force and effect.

Bid security in the amount of five percent (5%) of the Total Bid must accompany each Bid, and shall be made payable to the Owner.

The Owner reserves the right to waive any informalities or to reject any or all bids.

No bidder may withdraw his bid within forty-five (45) days after the actual date of opening thereof.

A Pre-Bid Conference is scheduled for June 15, 2010 at 10:00 a.m. at the office of GSA, Inc., 1022 S. Purpera Ave., Gonzales, LA 70737.

In addition to paper bids, electronic bids and electronic bid bonds for the followings project will be downloaded by the Ascension Parish Purchasing Department.
