    The petition of:
    BOONE PARSONS ENTERPRISES, L.L.C., a Louisiana limited liability company, authorized to do and doing business in the Parish of Ascension, State of Louisiana; represented herein by its duly authorized members, JASE Enterprises, L.L.C., acting through Thomas A.     Petitioner certifies that:
    A) Said tract of land was purchased on October 19, 2009 from Wilbert J. Sheets.
    B) Said tract of land is contiguous to the existing limits and boundaries of the City of Gonzales;
    C) Petitioner is the sole owner in fee simple of the entirety of the tract of land sought to be annexed; and,
     D) There are no resident property owners, nor registered votes, in the area proposed for annexation.

    As required by R.S. 33:172, Petitioner annexes hereto as Exhibit “B” a certificate by the Assessor of the Parish of Ascension certifying:
    That it is the record owner in fee simple of the tract of land sought to be annexed; and
    B) That said tract of land is assessed at the sum of TWENTY-TWO THOUSAND NINE HUNDRED AND NO/100 ($22,900.00) Dollars for the current year.
    4.
    As required by Louisiana R.S. 33:172, Petitioner attaches hereto as Exhibit “C” a certificate by the Registrar of Voters of the Parish of Ascension stating that no registered voters reside in the area sought to be annexed.

    Petitioner therefore requests that the City of Gonzales annex the above-described property by ordinance; that notice by publication be given once of the filing of the petition in the Gonzales Weekly Citizen; that said notice specify that anyone desiring to be heard with reference to the proposed ordinance shall notify the clerk or secretary of the City of Gonzales in writing and that the City of Gonzales, before adopting any such ordinance, shall grant such hearing; that if no hearing is requested or alternative after the required hearing, and after the expiration of ten (10) days following the publication of a notice as required by R.S. 33:172(B) said ordinance be adopted and the above tract of land annexed to the City of Gonzales.

    PROPOSAL
    ELECTRONIC TIME KEEPING SYSTEM
    The Parish of Ascension hereby issues a Request for Proposal (RFP) for an Electronic Time Keeping System. Proposals will be received by the Ascension Parish Government Purchasing Department, 120 East Railroad Street, Gonzales, Louisiana 70737 (mailing P.O. Box 2392, Gonzales, LA 70707), until 02:30 p.m. CST on  February 10, 2010.  RFP’s received after 02:30 p.m. will not be accepted.
    Overview
    A. Purpose.
    It is the purpose of this request for proposal to solicit for a Workforce Management solution.
    Vendors responding to this document should propose all services and associated costs to deliver a fully operational program to meet the needs of Ascension Parish.
    B. Vendor Qualifications
    Selection of the most suitable vendor and proposal is essential; therefore, proposals will only be accepted from vendors that demonstrate the following qualifications:
    1. Vendor must have been engaged in the business of time and attendance services for at least 10 years.  Please confirm
    2. Vendor must have demonstrated capability to implement the recommended proposal.  Please list other Louisiana government entities (state and local) using vendors solution
    3. Vendor must design, engineer, deliver, and support its own products.  Please confirm
    4. Vendor must be a Microsoft Gold partner or higher. Please provide response.
    5. Vendor must have in depth experience at installation, setup and training on their own Human Resource, Payroll, and Time and Attendance solution systems.  Please describe.
    6. Vendor must be able to provide Dell Server and SQL2005/2008 purchase options including 3 year Dell Gold support as part of a Dell OEM reseller offering.  Please provide response and options available to the parish.
    7. Vendor should provide warranty and support on the software and hardware that comprises the workforce management solution.
    Scope of Work
    Page 6 of 10 - A.  Must support the collection of data for up to 300 active employees, 55 managers, and 10 biometric magnetic strips and, Ethernet data collection terminals.
    B.  Must support the use of VMWare ESX 3.51.  Please list current support availability for VMWare
    C   Must allow continued punch collection at time clock if network is down
    D. Must Use Microsoft SSRS for reporting and reporting changes.  Please define how reporting is handled
    E. Must provide password protection and definition of multiple levels of user types and multiple security levels
    F. Must be able to track cost center transfers and shift differentials 
    G. Must provide in depth reporting of hours, schedules, attendance and missing punches
    H. Must track changes made to transactions in the system and be capable of
    providing audit trails of such changes
    I. Must have experience interfacing with all of the following financial systems:   Munis, New World, STW, Lawson, and Oracle.  Please provide a list of clients that currently have interfaces from vendor’s workforce management system to each listed financial solution 
    J.  Must have proven experience integrating with Kronos Human Resource and Payroll solutions within another Louisiana or Texas municipality.  Please list customer accounts and contacts where this type of integration has occurred
    K. Should support daylight saving without user intervention
    L. Should track employees by departments, groups, and divisions
    M. System must have ability to add an integrated telephone based data collection system for the collection of data from mobile employees
    N. Vendor must provide at least two Louisiana based governmental entities utilizing telephone based data collection
    O. Software should store a copy of biometric templates from time terminals  both at the terminal and in the database to prevent need to add employees again should clock have to be repaired or replaced;   Please describe how the system stores biometric templates
    P. Software must be web based and run using vendor provided JBOSS Application Server and either a vendor supplied Apache or IIS (provided with SQL) web server
    Q.  Vendor software shall provide for centralized download of terminal updates without manual intervention at the site of the time clocks
    R.  System must be able to handle historical edits and fully handle recalculation of pay as well as track for audit purposes
    S. Software must have option to add on an integrated absence management module that allows at the least the ability to track complex leave and attendance policies as well as to automatically issue mandated notifications and reporting while updating the timecard, schedule, and hours totals
    T. Vendors must provide Return on Investment profiles/reports from 3rd party research firms such as Nucleus Research or GFOA attesting to the savings afforded by customers through use of vendor’s applications.    Please submit two such reports including reference to vendor’s solution, customer, and outcome/benefit as a minimum

            Clock Restriction- (assigned clock group)
            Schedule restriction
            Access to Self Service Suring preset restricted times (such as shift change)
    V. Data Collection Terminals should be able to provide access to the following as configurable options: Totals, Schedules, Request time off, Messaging, Approve timecard, printing through the clock, review recent punches
    W. Data Collection Terminals must be designed to be used outside the parish network should the parish have need to setup a data collection device in a non- parish facility with internet access.  Please describe capabilities.
    Proposal Preparation and Submission Requirements
    To enable Ascension Parish to conduct a uniform review process of all responses to this solicitation, components of the proposal shall be submitted as set forth below. Ascension Parish reserves the right to reject submittals that do not follow the requested format. One original (duly marked) and five (5) copies of proposal are required.
    A. Cover Sheet.
    B. Management Summary (one page)
    Include a management summary which provides a statement of the vendor’s understanding of the services required by the Parish and an overview of how the vendor would provide these services to Ascension Parish.
    C. Vendor Profile
    Provide a Vendor Profile, to include:
    1. An overview of the company-address, telephone, and fax numbers; whether company is a local, regional, or national firm; areas of specialization.                                      
    2. The length of time the vendor has been in business under the present name
    3. An outline of the vendor’s background and overall qualifications.
    4. Company information as to all providers of software and hardware other than Vendor.  A minimum of 36 months availability of new, fully compatible replacement data collection hardware is expected
    D. Proposal Plan (three pages)
    1. Give a detailed, comprehensive presentation of the approach to be used by your firm to accomplish the tasks detailed in the Scope of Work.
    2. Specify any information you will require from the Ascension Parish Staff to enable you to provide this service,
    3. Present in detail, features and capabilities of the proposed application software. In addition, the vendor should include a description of any features (or value-added components) included or available in the proposed software that has not been specifically requested in the RFP.
    4. Provide a description of the SQL/Windows server System required to support the computing environment.
    5. Provide the following information:
    a. Hardware Environment – Describe the supported computer
    hardware environment in which the proposed software will run.
    b. Operating System - Identify the operating system that is supported by the proposed applications software and the proposed database management system in the hardware environment recommended above.
    Page 8 of 10 - c. List the operating system software support products required to support the recommended computing environment. List any additional vendor software products required to support your proposed application software.
    6. Provide Implementation Methodology –
    7. Provide training detail to include onsite training of up to 40    managers/timekeepers.
    8. Give a detailed description of the level and extent of all support to be provided after the implementation of the proposed software   Show any costs associated with this support in the cost quotations.
    E. Fee Structure (one page)
    1. All cost must be itemized in the vendor’s response; (e.g., license fees, annual support, training, installation, documentation, data collection, etc.)
    2. The vendor’s cost quotations must be valid for a minimum of sixty (60) days from the deadline for submission of proposals, and may not be increased once contract negotiations have been initiated by the parish.

    SCOPE OF SERVICES
    The Parish of Ascension and East Ascension Gravity Drainage District #1 (the Parish) have identified the need for wetland delineation and permitting consulting services (Services) on an as needed basis for 2010.  All individuals and sub-consultants proposing to work in any portion to provide these Services shall possess certifiable qualifications and experience with USACE Wetland Regulatory Permitting submittals and processes as defined by the current EPA Clean Water Act Regulations. In addition, the individual, consultant or sub-consultant must exhibit the ability and qualifications to perform wetland delineation acceptable by the USACE.

    The Parish of Ascension hereby issues a Request for Proposal (RFP) for Industrial Uniforms and Flat Supplies. Proposals will be received by the Ascension Parish Government Purchasing Department, 120 East Railroad Street, Gonzales, Louisiana 70737 (mailing P.O. Box 2392, Gonzales, LA 70707), until 02:00 p.m. CST on  February 10, 2010.  RFP’s received after 02:00 p.m. will not be accepted.
    Purpose of Proposal
    The Ascension Parish Government is requesting proposals from qualified vendors to provide rental uniforms, other clothing items and flat supplies to various Parish Departments located on the East and West Banks of Ascension Parish.
    Uniform Requirements
    General Type:
    65% Polyester
    35% Combed Cotton
    100 % Cotton
    All work shirts will be embroidered with custom Parish logo
    Ever person (except where noted) will have 11 uniforms assigned to them personally.  Five (5) at the beginning of the cycle that are ready to wear, one (1) in use and five (5) back at the contractors for cleaning, repairs, etc.
    Upon award vendor will visit each site with samples and take measurement.  Arrangements must be made with supervisors.
    Vendor would be allowed to charge back to Ascension Parish Government charges for lost clothing.
    Vendor will be responsible for all costs associated with having the uniforms available for wearing including:
    Proper fit   
    Any patches/emblem
    Pickup of soiled uniforms
    Drop off of cleaned uniforms
    No charge for size changes
    No charge for repairs
    No charge replacement due to wear
    Vendor will allow additional locations to be added to drop off and pick up locations at no additional cost.

    The term is for three (3) years beginning April 1, 2010.  NO PRICING CHANGES ARE ALLOWED DURING THE THREE (3) YEAR TERM.
